CARAFAP PROVIDES SUPPORT UNDER THE FOLLOWING WAIVERS:
- Family Support
- Community Pathway
- Community Support
- Family Support Waiver: Is designed to support and assist participants to live more independently in their homes and communities. The program provides a variety of support services that promote community living, including a self-directed service model and a traditional, agency-based service model. The services CARAFAP provides under this waiver include:
Services Provided Under the Family Support Waiver Includes the following:
- Family Peer & Mentoring Supports: Is designed to support and direct families to various community services and programs to assist the family with achieving a desired goal. It fosters connections and relationships which build individual and family resilience.
- Family Caregiver Training and Empowerment Services: Services provide education and support to the family caregiver that preserves the family unit and increases confidence, stamina, and empowerment. Education and training activities are based on the family/caregiver’s unique needs and are specifically identified in the person-centered plan
- Participant Education, Training & Advocacy Supports: Provide training programs, workshops, and conferences that help an individual develop self-advocacy skills, exercise civil rights, and acquire skills needed to exercise control and responsibility over other support services.
- Community Pathway Waiver: Provides comprehensive support in the home and community-based setting aimed to increase personal autonomy and self-advocacy across all settings. Additionally, individuals have the opportunity to learn and develop new skills, establish new relationships, and help them to identify personal interests.
- Community Support Waiver: Provides person-centered planning designed to support an individual receiving services under DDA Home & Community Based Services. Goals and outcomes are tailored to each individual based on his/her needs. This waiver provides life-long learning opportunities, recreational and leisure activities, and strengthens one’s personal development.
Services Provided Under the Community Pathway and Community Support Waiver Includes the following:
- Personal Supports: Personal supports enable waiver participants to accomplish tasks that they would normally do for themselves if they did not have a disability. Personal supports take the form of hands-on assistance (actually performing a task for the person) or cuing to prompt the participant to perform a task. Personal support is provided on an episodic or on a continuous basis.
- Community Development Services: Meaningful Day can occur in a group setting of 1 to 1 or 1 to 4 ratio size which is contingent upon an individual HRST. Under this model, CARAAP services individuals who are 18 and up. Such services are conducted in the community and CARAFAP for approved activities such as ZamDance or agency events.
- Transportation: The purpose of this program is designed to provide transportation services for non-medical appointments, obtain community access, and utilize third-party vendors such as Uber, Lyft, and/or metro.
- Respite: Services are designed to provide relief, aid, and emergency services to families of individuals with intellectual and developmental disabilities. This is a service where family members can obtain a sense of relief knowing that their children are safe and cared for with highly qualified and efficient services.
